Professor Enrico Croce, MD and et al ( M.Azzola, R. Russo, M. Golia and S. Olmi) , from the Scuola Speciale Ospedaliera A.C.O.I. Di Chirurgia Laparoscopica e Mini-Invasiva, Milano, Italy are presenting a case of a 69 year old patient diagnosed with a pulmonary lesion located in the right lower lobe by CT-scan. After a negative bronchoscopy the patient was scheduled for a thoracoscopic lung biopsy.

Once the trocars were placed, enlarged lymphnodes were noted upon exploration of the thoracic cavity.
The oblique fissure is identified and divided completely exposing the main pulmonary artery.

The right pulmonary artery is identified and dissected from the surrounded tissues. The branch to the right lower lobe is ligated with cut-gut....

...and then dived by using an ENDO-GIA stapler |

After the inferior pulmonary ligament was divided using electrocautery, the right lower lobe is elevated to provide adequate exposure of the right inferior pulmonary vein. this vessel is also ligated with plain cat-gut and divided by using an ENDO-GIA stapler.

The mediastinic pleura is opened and the right lower bronchus is dissected free. Once again an ENDO-GIA stapler is used to divide the bronchus taking care to avoid stenosis of the right middle lobe bronchus.

The right lower lobe is retrieved trough a mini-thoracotomy incision, protected with a disposable plastic bag. The pathologic report revealed "adenocarcinoma", with negative lymphnodes.

Cyanoacrylate is applied over the bronchial suture, to provide extra-strength.
