MINI INVASIVE TECHNIQUE FOR SAPHENOUS VEIN HARVESTING
The saphenous vein is usually the conduit of choice to bypass arterial blockages, to perform arterial bypass grafts and cardiac procedures.

Standard long skin incisions are usually performed to harvest the saphenous vein. This procedure is not free of wound complications such as erythema skin reaction, skin necrosis, wound infection (20%) and significant post operative pain.

The VASOVIEW, from Origin, allows saphenous vein harvesting using only 2 to 4 small incisions with obvious benefits such as less post operative pain, less scaring, infection rate and less hospital stay.
A 2 cm incision is made at the groin, and the great saphenous vein is identified. The tissue surrounding the vein is dissected completely until the adventitia is exposed.

The vasoview canula is connected to the camera.

The clear tapered tip of the canula provides direct visualization during the procedure while balloon inflation ensures atraumatic tissue dissection of the vein and side branches.

Between incremental advancements, the balloon is inflated and deflated every two centimeters. With the canula held stationary. The balloon inflation gently displaces surrounding tissue and effectively isolates the vein and side branches.

Once the canula is inserted anterior to the saphenous vein, the vein is identified as a white structure.

When the cannula reaches the knee, a two cm incision is performed to introduce the dissector, while the camera is slowly withdrawn.

A scope is inserted proximal and the isolated saphenous vein is clearly identified.

Air tight ports are used at the proximal and distal ports. The port’s balloons are inflated with 20 cm of air and the insuflation is maintained at 12-15mmHg .

A dissection ring is used to skeletonize the vein from surrounding tissues.
|

To complete the vein dissection, a 5-10 mm clip applier is used to ligate and divide the tributaries.
